Comprehensive ASD assessments for adults aged 18–65, designed for individuals seeking diagnostic clarification, academic accommodations, workplace support, or a better understanding of lifelong social, communication, sensory, or behavioural patterns.
Before Your ASD Assessment — Required Records
Psychological assessments.
Psychiatric assessments.
School accommodation documents (IEP, IPRC, academic accommodations)
Medication summaries, including medication history and recent blood work.
Hospital admission and discharge summaries.
Discharge summaries from residential, educational, or vocational placements
Specialized assessments and treatment summaries (genetic testing, OT reports, behaviour therapy reports, SLP reports)
Developmental History
Developmental history provided by a caregiver or close family member familiar with early development
Documentary evidence: previous assessments, school reports, report cards or teacher observations
