What is Tinnitus?

Tinnitus is the perception of sound without an external source, meaning others cannot hear it. While commonly described as a ringing noise, it can also sound like buzzing, roaring, clicking, hissing, or whistling.
Tinnitus affects an estimated 10% to 25% of adults and can also occur in children. For some, the symptoms may improve over time. For others, tinnitus may persist or even worsen. When it lasts for three months or longer, it is considered chronic tinnitus.

Types of Tinnitus

Objective Tinnitus

A rare form caused by physical sound-producing processes within the body, such as vascular, muscular, or skeletal functions. The most common type, pulsatile tinnitus, occurs in sync with the heartbeat and may be detectable by a doctor.

Subjective Tinnitus

The most common type, where no external sound source can be identified. It is often linked to changes in the auditory system, such as hearing loss or nerve dysfunction.

Synptoms of Tinnitus

Tinnitus symptoms vary from person to person.
You may experience:

  • Phantom sounds in one or both ears or inside your head.
  • Ringing, buzzing, roaring, clicking, whistling, or hissing noises.
  • High- or low-pitched sounds that can be intermittent or constant.
  • Sounds that change when you move your head, neck, or eyes 
(somatosensory tinnitus).

Causes of Tinnitus

While the exact causes of tinnitus are not fully understood, several factors have been linked to its development:

Hearing Loss

Tinnitus often accompanies hearing loss due to aging or noise exposure. Damage to the inner ear (cochlea) disrupts how sound signals are processed by the brain, leading to “phantom” sounds. The brain adapts by becoming more sensitive to internal signals, creating the perception of ringing or buzzing.

Noise Exposure

Loud environments, such as concerts, construction sites, or military settings, can cause noise-induced tinnitus. It is a common condition among veterans.

Head or Neck Injuries

Trauma to the head, neck, or auditory nerve can interfere with sound processing and lead to tinnitus.

Medications

Certain medications, such as high doses of NSAIDs (ibuprofen, aspirin), some antibiotics, cancer treatments, and antidepressants, can trigger or worsen tinnitus.

Earwax Buildup or Infections

Blockages in the ear canal from earwax or fluid buildup can cause temporary tinnitus.

Treatment Options for Tinnitus

If tinnitus results from an underlying condition, addressing the cause—such as removing earwax or treating a jaw disorder—may relieve symptoms.
For cases with no identifiable cause, treatments focus on symptom management:

Conventional Treatments

Sound Therapy

Uses external sounds (white noise, nature sounds, or hearing aids) to mask tinnitus, retrain the brain, and reduce the perception of the ringing.

Behavioral Therapy
Counseling and cognitive behavioural therapy (CBT) help individuals manage emotional distress and improve quality of life.

Medications
While no specific drug treats tinnitus, doctors may prescribe antidepressants or anti-anxiety medications to improve mood and sleep.

Emerging Treatments

A safe, non-invasive brain stimulation technique that uses magnetic pulses to stimulate specific areas of the brain. Some studies have shown moderate symptom relief, though further research is needed to confirm its long-term effectiveness.

Electric Stimulation

Researchers are testing non-invasive electrical stimulation of the inner ear to suppress tinnitus without affecting normal hearing.

Deep Brain Stimulation (DBS)

An invasive procedure that implants electrodes in the brain to regulate neural activity. While early results are promising, more studies are required.

A non-invasive technique using mild electrical currents to target brain areas involved in tinnitus perception, such as the dorsolateral prefrontal cortex (DLPFC) and auditory cortex. Some studies report moderate to significant improvement.

A safe, non-invasive technique that uses light therapy to target the inner ear or brain with minimal side effects. While some studies suggest it may provide short-term symptom relief, the long-term benefits are still not well established.
